As a society interested in the welfare of animals, PAWS has assumed responsibility for the welfare of every animal that enters our shelter. Our purpose is to place an animal in a responsible home where it will receive veterinary care when needed, where it will be controlled so as not to become a free roaming animal, or add to the surplus of animals already in the community.
To insure these ends, we have formulated this application form to assist our staff in the careful selection of home placements for our pets available for adoption. Only those persons who are adults of legal age and obtain a satisfactory application will be eligible. In order to be considered for our fostering program you must have the knowledge and consent of all adults living in your household, have a valid photo ID with current address, and understand a PAWS Humane Society staff member must approve your application.
By submitting this form you are certifying the information you have given is true and recognize any misrepresentation of facts may result in losing the privilege of fostering or keeping a fostered pet. You understand that the PAWS Humane Society has the right to deny your request to foster an animal and authorize investigation of all statements made in this application. You understand this application is the property of the PAWS Humane Society of Floyd County, IA.
